What we do

Key Transitions Ltd understands the transition from residential care to independent living can be extremely stressful and daunting for young people. We have developed a service that will support young people through this difficult period of transition. We place equal importance on practical and emotional matters viewing them as intrinsically linked. We support care leavers in building and developing a range of essential skills and the tools to progress more positively into independent living.
The service provided by Key Transitions Ltd is available for young people aged 16-25 who may be subject to after care services provided by Social Services. All services will be developed according to the specific needs of the young person and will be produced in accordance with the local authority placing them in our care.

The service provides semi-independent accommodation with support. We help young people understand what they need to do to sustain a tenancy.

Young people are encouraged to develop essential skills to maintain a home, budgeting and money management advice.

We encourage day-time activities through training, employment, education or volunteering in preparation to move into longer term accommodation. After young people have moved out, we are able to provide a resettlement service to support their move.

Support is delivered through regular individual sessions with a dedicated support worker. Young people have an agreed support programme to help them achieve personal goals and aspirations.

 

We help young people move on to:

  • Shared accommodation
  • Private lettings
  • Local authority accommodation
  • Registered social landlords
  • Other housing providers

Young people can be referred to Key Transitions through the Local Authority:

  • The Leaving Care Team
  • Homeless Person Unit
  • Youth Offending Team

UASC Support from Key Transitions Ltd Key Transitions have experience of working with Local Authority UASC teams to provide suitable placements for all young people referred to us and seek to place them in comfortable, independent accommodation in a location appropriate to their needs.
